Yes we do see patients covered by workers compensation.  All Workers Compensation must supply our office with the following information:

  • Your Employers name and Address
  • Your Insurers Name and Address – we need the address of the office your insurance claim is going through as some insurers have multiple offices.
  • Your Claim number
  • Date of Injury
  • Case manager’s name and direct phone and fax numbers.

It is the patient’s responsibility to provide this information – if this information is not available the patient will be charged the Workers Compensation fee and will need to claim this back from their insurer.

We are happy to bill directly to the insurer if the information is provided, however if the insurer denies the claim or does not pay in full the patient is liable for the account.

All workers Compensation patients must have an approval letter from the insurance company prior to each consultation.
